Continuum of Care
VIP provides Care Coordination to its patients and also those in the community through collaborative relationships.
VIP offers supportive housing and shelter services to clients with mental health, substance abuse, chronic homelessness, HIV/AIDS, and other chronic illnesses. Additionally, community residents benefit from VIP’s affordable housing stock.
HIV and Hepatitis C Testing
VIP offers preventative testing for HIV, Hepatitis C, and other illnesses to reduce the number of people that become infected and to improve the overall health of the Bronx community.
VIP offers primary care services to patients across the age spectrum. Patients receive chronic care management and preventative to keep them healthy and out of the hospitals.
VIP offers integrated behavioral health with primary care. Patients receive mental health and substance use services with close monitoring of medical conditions. Ancillary services including outreach, education, mobile crisis, and case management are also provided.
Access to Care
VIP services are easily accessible to all patients regardless of their ability to pay. On call and mobile crisis services allow for 24-hour access to all services.
